Actually I'm fairly sure the 1h/2h cav ghost reach + missing when it is totally hitting impressions are due to swing height and the overall funky hitboxes of people when hitting above the head. Mounted swing animations are divided into multiple heights that all have different trajectories, out of which the most horizontal one has greatest horizontal reach (makes sense). The problem is that even though that one looks like it isn't hitting anything because too high, it does connect with heads. Inversely, mounted swings missing even when you clearly hit are often due to swinging too low.
