This map has the worst spawns, attackers spawn inside after first few deaths, we are removing from our server.

Author should check entry point 32. AFAIK, it marks the attackers "base". Every attacker will spawn there at round start. Afterwards, a complex system of distance and occupation calculation determines attacker spawn. Generally, if the spawns are not clearly separated by distance (entry 0 or flag and entry 32 being used as "bases" in the calcs), strange things might happen.
