Game Freeze Problems

My computer conf.

Intel i5 320GHz (4CPUs)
4GB RAM
Geforce GTS 250

My game freeze and take 5 - 7 second lag especially spawn times. Its big performance problem for me, my game configs set to % 106 but always feel this lag. Duel or 10 player or 200 player always lag. Its fixed 2 patch ago but now same lags again. Its not internet lag, its software lag. I feel lag only CRPG mod. I play with 200-300 player other mods but not feel lag.

I fix this problem...

All settings full and lags out - fans slow... What a game.

check or uncheck load textures on demand
enable Vsync
enable max fps, set to 60-70
turn off shadows
turn off unlimited body count/corpses
