Random disconnection

I get randomly disconnected from server without a logical reason (saw that few people have the same issue)
It doesn't happen after a lag or something, everything is fine I'm killing, my multiplier is high and bam! disconnected from server.
If I try to rejoin, it says there is a player with that name. I absolutely have no idea what is causing that and its obviously not hardware related.

I had this problem a while back and it turned out to be my old cable modem. You could try checking your modem's logs, if possible, to see if it's reconnecting periodically.
