3. Time and server, as accurately as possible: June 1st 1:29 P.M
4. Written description of what happened, the whole story. Also what happened before and after.: He charged in and died, reconnected and then dies almost immediately after he double spawned.
5. Why you think the offender did what he did.: Because he wanted another chance at life

He was banned recently for teamkilling, although it was for a short time. 12 hour ban for double spawning.
