Written description of what happened, the whole story. Also what happened before and after: He said he is going to kill 10 people this round and died without a kill. I t-bagged him together with Nord_Jerv. He revived himself but still failed to get 10 kills in that round. 5. Why you think the offender did what he did: Died too early. 6. Multiple Screenshots:

Pretty obvious Phase spawned twice in one round. You are probably one of the most experienced guys in this game, and you know this is against the rules.
