4. Written description of what happened, the whole story. Also what happened before and after. : First It was starting the battle and i accidentaly hitted him with my stick, than he kept attacking me for all the rounds for no reason at all, he also attacked other players.
5. Why you think the offender did what he did. : Because he is a freaking team killer...

7. Names of players who witnessed what happened. : Well the other player that he killed.

I can confirm this. The screenshot actually shows the second teamkill. I witnessed both.
