That what happened to me yesterday. Joined a server, checked if all my gear is still there and noticed that my Desert Turban is a Pumkin head. Of course I spawned with it, admired myself for about 15 seconds and then CTD.

You need to enable WSE, and then use ingame option: show banner outlines - "on friendlies" (or "on everyone" and then u'll see also a red circles around enemies banners)
