I had a variant of this. First it was real large spikes where the image on my screen froze still for a good 2 minutes but I could still hear sound. I lowered the settings and it stopped.
But now, I have an issue where I get lag spikes that are about 2 to 5 seconds long where everything is choppy so I have to back off any fight I am in or I will surely lose.

I'm getting the same, usually when there are a lot of people in a server, say 70+ in a siege server. I might try lowering my settings again when playing in crowded servers, see if it helps.

Same here even at 15 people in duel.

Been having the same problems myself, although when checking the task manager the process is eating up far more then it should. One time it was at 1.6mil and another time at 600,000 for memory usage. Though to be honest I've been meaning to format C\: for a while and reinstall windows could just be something that happened to me, although I was playing earlier and other then everything feeling sluggish it was fine.

Dammit its back again :?
Same lagg like last time for some odd reason..

When you load up the game, try turning off "Load Textures on Demand". It cut it down a lot for me, though it still happens a little bit.

The lag spikes you're experiencing may be of a different kind to what I had, but work worked for me was: Run > services.msc > right click Wireless Zero Configuration > Stop. I do that everytime my computer starts up, of course it only applies if you're using wireless. (and just to note; i'm using Windows XP, I don't know if this only applies to XP)

I get it also, and it also started after the last patch for me to. It's really annoying, every first fight or 2 is bad lag spikes, then its random for the rest of the round. The spawn lag is bad too.
