Well. Imma start out by saying that im getting 70 FPS ingame. Now I get this strange frame rate thingy lag or something. . Its saying im getting 70 FPS but my game is like Stuttering. I can't fully explain it cuz I dont know the perfect word for what its doing. I have V-sync turned off. I dont know what else it could be... Please help When i stop moving its fine, I can swing perfectly with out a problem... But when i start to move it starts stuttering again..

Samething for me but it is since 2 patch ago or something like that. My friend geekiller had also the same issue. He solved it by buying a new computer :(

theres no reason to have vsync off if you have a decent computer and you should also have max fps set to 60 or 70 so you dont burn out your video card.
you've listed NO steps you've take to remedy problem. Do them 1 at a time so you know what the problem was and what fixed it
ensure computer is not overheating
Update video drivers
enable or disable load textures on demand

There is one though: When I turned Vsync on I had an amazingly stable and good FPS, but the melee combat was totally messed up, blocks were not in time, etc.
Just limiting the FPS would be the better solution, steer clear of Vsync in this game.
I agree about the other parts though.

If you have an nvidia graphics card and the latest drivers, go into the nvidia control panel -> adjust desktop size and position -> change the dropdown menu where it says "Perform scaling on:" to Display.
This sounds like the exact problem I was having, where I had anywhere up to 140 fps but the game was still stuttering. Turning off the new scaling feature on the nvidia drivers using the steps above worked for me; if it doesn't work for you try changing some more of the settings on that page, e.g. change the scaling mode to Full Screen or No Scaling, and turn off "Override the scaling mode set by games and programs".
