How do these work? Do they add a amount to your "universal" armour rating, or do they only count when that part of the body is hit?

Greaves only apply to your legs and helms only apply to your head.
Gauntlets add to your body armor.
So for simplicity sake, you have three target areas: Head, Legs, Torso/arms.
Helmets only protect your head.
Greaves only protect your legs.
Body armor/gloves protect the torso/arms.

feet you may be able to skimp on, but do not skimp on a helm.
I laugh at guys in field plate that wear no helm for looks. i can 1shot them with an overhead to the face

Rarely, body armor adds head armor as well as body and leg armor (gothic plate with bevor).
